This package includes: - 75 minute initial consultation - Annual wellness and hormone bloodwork and lab review - 3 month checkup with lab work and brief telemedicine lab review - 6 month checkup with lab work and brief telemedicine lab review This package, valued at $1,404, is offered at a discount if prepaid. Take charge of your hormonal health by booking an Initial Male Hormone package today. We must contact you and collect fees prior to confirming your appointment. Please allow 72 BUSINESS HOURS before your desired appointment request. This allows us to confirm and you to complete necessary documents for review prior to your appointment. Dr. Miller is knowledgeable in helping men improve their hormones to gain optimum testosterone status and support hormone balance. This package is for initial patients and includes a hormone focused initial evaluation, initial wellness and hormone bloodwork and an initial therapeutic plan. It also includes a 3 month followup bloodwork panel with a telemedicine visit to review your progress and symptoms, as well as a 6 month bloodwork evaluation of your hormones and wellness labs and a telemedicine visit to make sure you are on the right track. Once you are on the correct therapeutic dose you can then purchase visits and labs a la carte or purchase the maintenance hormone plan for testosterone. Federal law requires providers to have a limited 6 month prescription for testosterone as it is a controlled substance. Your maintenance program would include one annual visit with annual labs and a six month interval lab check with a brief telemedicine checkin after the lab results are available. If you prefer to see the Physician Assistant there is a discount on the package of $119.

Cancelation Policy

Appointments cancelled less than 48 business hours prior to the day/time which are not rescheduled may be subject to a cancellation fee. If it is less than 48 hours and you need to reschedule, please call the office 770-676-1027.
